Immuuncheckpointremmers (ICIs) plus platina-gebaseerde chemotherapie is een standaard eerstelijns combinatie voor metastatisch niet-kleincellig longcarcinoom (mNSCLC). De optimale behandeling na progressie op deze therapie is niet duidelijk. De fase 2-studie SCORPION, in acht centra in Japan heeft tweedelijns docetaxel plus ramucirumab (DTX-RAM) na progressie op ICIs plus platina geëvalueerd. Prof. Makoto Ishii (Universiteit van Nagoya) en collega’s publiceren de studie in eClinicalMedicine.¹

De studie includeerde 33 mNSCLC-patiënten (mediane leeftijd 66 jaar; range 42-79; ECOG performance status 0 of 1) met ziekteprogressie na eerstelijns ICI plus platina. De patiënten kregen intraveneus DTX 60 mg/m² plus 10 mg/kg RAM op dag één van drie-weekse cycli. Het primaire eindpunt was objective response rate, met nul- en alternatieve hypothese 10% respectievelijk 30%. De figuur laat zien dat elf patiënten partiële respons hadden, voor een ORR van 34,4% (80%-bti 23,1-47,2) waarmee het primaire eindpunt bereikt werd. De mediane progressievrije overleving en overall survival waren 6,5 maanden respectievelijk 17,5 maanden. Graad 3 of 4 anemie en febriele neutropenie werden gezien in twee (6%) respectievelijk drie (9%) patiënten. Er waren geen graad 5 treatment-related adverse events.

De onderzoekers concluderen dat onder mNSCLC-patiënten met progressie na eerstelijns ICI plus platina-gebaseerde chemotherapie, DTX-RAM bemoedigende antitumorwerkzaamheid had met een manageable veiligheidsprofiel.
