De studie includeerde 132 patiënten (47 vrouwen en 85 mannen, mediane leeftijd 63 jaar) met ECOG performance status 0 of 1 in 25 centra. De patiënten hadden progressie na eerstelijns bevacizumab plus fluorouracil met irinotecan of oxaliplatin. Ze werden gerandomiseerd naar arm A (voortzetting van bevacizumab met FOLFIRI of mFOLFOX6; n=65) of arm B (cetuximab plus FOLFIRI of mFOLFOX6; n=67) waarbij de tweedelijns chemotherapie werd gekozen als crossover van de eerstelijns chemotherapie. Het primaire eindpunt was het percentage progressievrije patiënten na vier maanden.
Deze figuur toont de PFS in beide armen en deze figuur toont de overall survival, voor alle patiënten en voor patiënten met verschillende moleculaire kenmerken van de tumoren. De vier-maands PFS was 80,3% (95%-bti 68,0% tot 88,3%) in arm A versus 66,7% (95%-bti 53,6% tot 76,8%) in arm B. De mediane PFS was 7,1 maanden in arm A versus 5,6 maanden in arm B (HR 0,71; p=0,06). De mediane OS was 15,8 maanden in arm A versus 10,4 maanden in arm B (HR 0,69; p=0,08).
De onderzoekers concluderen dat de studie geen significant verschil tussen beide armen heeft laten zien, maar dat de resultaten van de studie wel suggereren dat continuering van bevacizumab met switch van chemotherapie resulteert in betere uitkomsten dan switch naar cetuxinab en switch van chemotherapie.

Summary: The French multicenter PRODRIGE18 study included patients with first progression of KRAS-wt metastatic colorectal cancer after bevacizumab plus fluorouracil with irinotecan or oxaliplatin. The patients were randomized to arm A (FOLFIRI or mFOLFOX6 plus continuation of bevacizumab) or arm B (FOLFIRI or mFOLFOX6 plus cetuximab); the second-line chemotherapy was chosen as crossover from the first-line chemotherapy. The study showed a non-significant PFS and a non-significant OS advantage for arm A, suggesting that continuation of bevacizumab with switching of chemotherapy might be the preferred option after progression of mCRC on first-line bevacizumab plus chemotherapy.
