Pelvisch
recidief is een frequent patroon van recidief onder vrouwen met
endometriumcarcinoom. Een multicenter gerandomiseerde studie van de Amerikaanse
NRG Oncology/Gynecologic Oncology Group heeft toevoeging van cisplatine aan
radiotherapie (RT) voor deze patiënten geëvalueerd. Prof. Ann Klopp (MD
Anderson Cancer Center, Houston TX) en collega’s publiceren de studie in het Journal of
Clinical Oncology.1
De studie
includeerde 165 patiënten met laaggradige ziekte met endometrioïde histologie
(82%) en recidief beperkt tot de vagina (86%). De patiënten werden 1:1 gerandomiseerd naar alleen
RT of RT met concurrent cisplatine 40 mg/m2 eens per week. Acute
toxiciteit was meer frequent in de chemoradiotherapiegroep dan in de alleen-RT
groep. De mediane progressievrije overleving was numeriek langer in de
alleen-RT groep (niet bereikt) dan in de RT plus cisplatinegroep (73 maanden;
HR 1,25; 95%-bti 0,75-2,07). Na drie jaar behandeling waren 73% van de
patiënten in de alleen-RT groep en 62% van de patiënten in de RT plus cisplatinegroep
progressievrij en in leven.
De
onderzoekers concluderen dat onder vrouwen met lokaal recidief van
endometriumcarcinoom uitstekende uitkomsten bereikt kunnen worden met alleen
radiotherapie.

recurrent endometrial cancer, excellent outcomes can be achieved with radiation
therapy alone, whereas addition of cisplatin to radiation therapy did not
improve progression-free survival.
