Chirurgisch ingrijpen bij colorectale lever metastasen resulteert in een vijfjaars-overleving van ongeveer 40%. Toevoeging van oxaliplatin- en fluorouracil-chemotherapie leidt tot verlenging van de PFS. Bij patiënten met gevorderde ziekte en KRAS exon 2 wild type tumoren resulteert toevoegen van cetuximab aan chemotherapie in langere overleving. Prof. John Primrose (University of Southampton) en collega’s publiceren vandaag online in Lancet Oncology uitkomsten van de EPOC studie naar toevoeging van cetuximab aan chemotherapie bij patiënten met resectabele colorectale levermetastasen.¹

Tussen 26 februari 2007 en 1 november 2012 werden patiënten met KRAS exon 2 wt resectabele tumoren gerandomiseerd naar uitsluitend chemotherapie (n=128) of cetuximab plus chemotherapie (n=129). De mediane follow-up was 21,1 maanden in de chemotherapie-arm en 19,8 maanden in de chemotherapie plus cetuximab-arm. Na een overall mediane follow-up van 20,7 maanden bedroeg de PFS 14,1 maanden in de cetuximab-arm tegen 20,5 maanden in de uitsluitend chemotherapie-arm (HR 1,48; p=0,030). De moleculaire basis voor deze onverwachte uitkomst dient te worden onderzocht, maar ‘at present the use of cetuximab in this setting cannot be recommended’, concluderen de auteurs.
