Het
Amerikaanse National Comprehensive Cancer
Network noemt aanwezigheid van extracapsulaire verspreiding en/of positieve
marges in orofarynxcarcinoom als indicaties voor toevoeging van chemotherapie
aan postoperatieve radiotherapie. Deze aanbeveling berust op wat het NCCN
omschrijft als high-levelevidence. Prof. Jay Piccirillo
(University of Washington in St. Louis MO) en collega’s publiceren online in Cancer een kritische beschouwing van deze evidentie.1 De auteurs wijzen op (in
hun ogen) een groot aantal tekortkomingen. Ze stellen dat de literatuur waarop
de richtlijn gebaseerd is geen high-level
evidence biedt, en dat de relevantie van toevoeging van chemotherapie aan
adjuvante radiotherapie voor HPV/p16-positief orofarynxcarcinoom onduidelijk
blijft.
