Lapatinib in combinatie met capecitabine of letrozol is een geaccepteerde behandeling voor trastuzumab-resistent HER2-positief mammacarcinoom (MBC). Er is echter geen duidelijkheid over de werkzaamheid van deze behandeling in patiënten die eerder pertuzumab en ado-trastuzumab emtansine (T-DM1) hebben gekregen. Een studie van MD Anderson Cancer Center (Houston) heeft deze werkzaamheid onderzocht. Dr. Rashmi Murthy en collega’s publiceren de studie online in Breast Cancer Research and Treatment.¹

De studie includeerde 520 patiënten die tussen begin 2003 en eind 2017 in Houston lapatinib-gebaseerde therapie kregen voor HER2-positief MBC. Onder deze patiënten waren er 43 die eerder pertuzumab of T-DM1 hadden gekregen (‘target cohort’) en 477 die niet eerder pertuzumab of T-DM1 hadden gekregen (‘comparison cohort’). De clinical benefit rate op lapatinib-gebaseerde therapie was 28% (95%-bti 10-32) in het target cohort en 40% (95%-bti 36-45) in het comparison cohort. De mediane duur van behandeling met lapatinib was 5 maanden (95%-bti 3,0-9,0) in het target cohort en 6,7 maanden (95%-bti 5,9-8,0) in het comparison cohort. In beide cohorten waren mediane tijd tot progressie en overall survival langer in de groep patiënten met de novo metastatische ziekte dan in de groep patiënten met ziekterecidief.

De onderzoekers concluderen dat lapatinib-gebaseerde therapie een actieve therapeutische optie is voor HER2-positief MBC, ook na eerder trastuzumab, pertuzumab en/of T-DM1.
