De standaard
behandeling na maximale veilige resectie voor glioblastoom in concomitante
radiotherapie en chemotherapie. Deze behandeling wordt niet goed verdragen door
sommige patiënten met slechte performance status. Een analyse van de National
Cancer Database heeft de waarde onderzocht van sequentiële radiotherapie en
chemotherapie voor deze patiënten. Dr. Chi Zhang (University of Nebraska
Medical Center, Omaha) en collega’s publiceren de analyse online in het Journal of
Neuro-Oncology.1
In de
NCDB-2015 identificeerden de onderzoekers 84 patiënten met een Karnofski
performance status tussen 10 en 70, die resectie ondergingen voor glioblastoom,
gevolgd door alleen radiotherapie (n=62; 73,8%) of sequentiële radiotherapie en
chemotherapie (n=22; 26,2%). Er was tussen deze twee groepen geen significant
verschil in overleving (p=0,84). Er waren 31 patiënten in de leeftijd 70 jaar
of ouder; onder deze patiënten was hogere leeftijd geassocieerd met slechtere
overleving (HR 1,06 per jaar; p<0,0001) ongeacht KPS.
De
onderzoekers concluderen dat sequentiële radiotherapie en chemotherapie
vergeleken met alleen radiotherapie na resectie voor glioblastoom in patiënten
met slechte performance score geen overlevingsvoordeel biedt.
