De
multinationale fase 3-studie KEYNOTE-048 evalueerde eerstelijns pembrolizumab
voor recidiverend of metastatisch squameus celcarcinoom van hoofd en hals (R/M HNSCC).
Patiënten werden gestratificeerd volgens PD-L1 combined positive score (CPS) 1:1:1 gerandomiseerd naar
pembrolizumab monotherapie (pembro-mono), pembrolizumab plus chemotherapie
(pembro-chemo), of cetuximab plus chemotherapie (cetux-chemo). In 2019 is de
primaire analyse van overall survival
onder alle patiënten en in de groepen met CPS 20 of hoger en CPS1 of hoger gepubliceerd. Dr. Nancy Lee (Memorial Sloan Kettering Cancer Center, New York)
en collega’s publiceren nu in JAMA
Oncology een analyse van uitkomsten van patiënten met intermediaire CPS (1-19) en lage CPS
(<1).1
De analyse
laat zien dat de CPS geassocieerd was met overall
survival onder patiënten in de pembro-mono en de pembro-chemogroepen, maar
niet in de cetux-chemogroep. Onder patiënten met intemediare CPS was de OS met
pembro-chemo (HR 0,69; p=0,01) maar niet met pembro-mono (HR 0,86; p=0,25)
beter dan met cetux-chemo. Onder patiënten met lage CPS was noch pembro-chemo
(HR 1,13; p=0,60) noch pembro-mono (HR 1,4; p=0,14) geassocieerd met betere OS
vergeleken met cetux-chemo. De progressievrije overleving was onder patiënten
met intermediaire CPS similar met
pembro-chemo en cetux-chemo (p=0,51), en korter met pembro-mono dan met
cetux-chemo (HR 1,55; p=0,001). Onder patiënten met lage CPS was pembro-chemo
geassocieerd met trend naar kortere PFS (HR 1,42; p=0,12) en was pembro-mono
geassocieerd met significant kortere PFS (HR 2,17; p=0,001) dan cetux-chemo.
De
onderzoekers concluderen dat de analyse suggereert dan CPS geassocieerd was met
OS onder patiënten die pembro-chemo en pembro-mono kregen, maar geen algemene
prognostische factor was in R/M HNSCC.
