De Cellular Dissociation Grade (CDG), gebaseerd op tumor budding en cell nest size in pretherapeutische biopten en primaire resecties van slokdarm squameus celcarcinoom (ESCC), is een
onafhankelijke voorspeller van de prognose. Een studie van de Technische
Universität München heeft de prognostische impact van CDG onderzocht in ESCC na
neoadjuvante therapie. Prof. Wilko Weichert en collega’s publiceren de studie online in het British Journal of
Cancer.1
De studie
includeerde 122 ESCC-patiënten die pretherapeutische biopsie ondergingen en
vervolgens neoadjuvante therapie kregen. De CDG voor individuele patiënten
bleef stabiel tussen pretherapeutische biopten en post-neoadjuvante
resectiespecimina. De CDG was geassocieerd met therapierespons, en was in
univariate analyse een sterke voorspeller van overall survival, ziektespecifieke overleving, en ziektevrije
overleving. In multivariate analyse was CDG naast metastase de enige
significante voorspeller van DSS en DFS. In vergelijking met CDG-graad 1
carcinomen was de multivariate DFS-HR 3,3 voor CDG-G2 en 4,9 voor CDG-G3
(p=0,016).
De
onderzoekers concluderen dat post-neoadjuvante CDG prognostisch was voor
therapierespons en overleving in ESCC.
