Immuuncheckpointblokkade (ICB) in combinatie met chemotherapie is een standaard-behandeling voor extensief-stadium kleincellig longcarcinoom (SCLC). In niet-SCLC zijn specifieke genomische veranderingen zoals in KEAP1 en STK11 geassocieerd met resistentie tegen ICB. Tot op heden is geen genomische verandering bekend die geassocieerd is met resistentie tegen ICB in SCLC. Een retrospectieve cohortstudie met validatie in het cohort van de prospectieve CheckMate 032-studie heeft de associatie tussen mutatie in het retinoblastoom 1 (RB1)-gen en werkzaamheid van ICB voor SCLC geïnventariseerd. Dr. Afshin Dowlati (Case Western Reserve University, Cleveland OH) en collega’s publiceren de studie in JCO Precision Oncology.¹

De retrospectieve cohortstudie includeerde 42 SCLC-patiënten die ICB-monotherapie of –combinatietherapie kregen. De groep patiënten met RB1-wildtype had een mediane overall survival 23,1 maanden (95%-bti 9,0-37,5), versus 5 maanden (2,5-26,0) in de groep met RB1-mutaties (p=0,04). DNA- en RNA-sequencing liet zien dat RB1-mutatie geassocieerd was met downregulatie van immuun-gerelateerde genen en een immuun-exclusie fenotype. Onder de 460 SCLC-patiënten in CheckMate 032 (nivolumab met of zonder ipilimumab) hadden RB1-wildtype patiënten significant betere OS met nivolumab dan RB1-mutant patiënten (HR 1,41; p=0,041). Patiënten met lagere RB1 loss-of-function signatuurscores hadden langere OS met nivolumab dan patiënten met hogere scores.

De onderzoekers concluderen dat SCLC-patiënten met RB1-wildtypestatus of lagere RB1 loss-of-function signatuurscores betere uitkomsten hadden met ICB-monotherapie dan patiënten met RB1-mutaties of hogere scores.
