Tumormutatiebelasting
(TMB) wordt onderzocht als predictieve biomarker van werkzaamheid van
immuuntherapie voor niet-kleincellig longcarcinoom (NSCLC). De multinationale
fase 3-studie BFAST onderzocht veiligheid en werkzaamheid van eerstelijns
gerichte therapieën of immuuntherapie voor niet-resectabel stadium IIIB of IV
NSCLC. Cohort C van de studie evalueerde bloed-gebaseerde TMB (bTMB) als
biomarker van werkzaamheid van atezolizumab. Prof. Solange Peters (Universiteit
van Lausanne, Zwitserland) en collega’s publiceren de resultaten van cohort C in Nature
Medicine.1
Het cohort
telde 471 patiënten met bTMB ≥ 10 mut/Mb, die 1:1 werden gerandomiseerd naar
eerstelijns atezolizumab of platina-gebaseerde chemotherapie. Het primaire
eindpunt was progressievrije overleving in de groep patiënten met bTMB ≥ 16
mut/Mb. Panel a van de figuur laat zien dat voor dit eindpunt het verschil tussen de atezolizumabgroep
en de chemotherapiegroep niet significant was (HR 0,77; p=0,052). Panel b laat
zien dat de overall survival onder de
patiënten met bTMB ≥ 16 mut/Mb evenmin significant verschilde tussen beide groepen
(HR 0,87; 95%-bti 0,64-1,17) maar dat in de atezolizumabgroep een hoger
percentage patiënten lange-termijn overleving had. Adverse events die leidden tot discontinuering van de behandeling
werden gezien in 10% van de patiënten in de atezolizumabgroep en 20% van de
patiënten in de chemotherapiegroep.
De
onderzoekers concluderen dat cohort C niet het primaire eindpunt bereikt heeft.
Er was wel een hoger percentage patiënten met lange-termijn overleving met
atezolizumab versus chemotherapie onder patiënten met hoge bTMB.

The cohort did not meet the primary end point of progression-free survival in
the population with bTMB ≥ 16.
