Op CD19
gerichte CAR T-celtherapie voor B-cel ALL heeft geresulteerd in hoge
percentages patiënten met complete remissie (CR), maar in sommige patiënten is
ontstaan van CD19-negatieve leukemie gezien. Het is denkbaar dat bispecifieke
CAR T-celtherapie gericht op zowel CD19 als CD22 het risico van CD19-negatief
recidief kan verlagen. Een pilot studie in China heeft de feasibiliteit van
deze benadering onderzocht. Prof. Weidong Han (Ziekenhuis van het Chinese
Volksleger, Beijing) en collega’s publiceren de studie online in het Journal of Hematology & Oncology.1
De studie
includeerde zes volwassen patiënten met recidiverend of refractair B-ALL die autologe
bispecifieke CD19/CD22 CAR T-cellen kregen toegediend in een dosering
uiteenlopend van 1,7 x 106 tot 3,0 x 106 cellen per kg
lichaamsgewicht. De behandeling resulteerde in robuuste cytolytische activiteit
tegen de targetcellen. In zes van zes patiënten werd MRD-negatieve CR bereikt.De CAR T-cellen werden gedetecteerd in bloed,
beenmerg, en cerebrospinaal vocht van de patiënten. Er werd geen
neurotoxiciteit gezien. Eén patiënt had vijf maanden na de behandeling recidief
met blastcellen zonder expressie van CD19 en verminderde CD22 expressie op het
blastoppervlak.
De
onderzoekers concluderen dat autologe CD19/CD22 CAR T-celtherapie feasible en veilig was en resulteerde in
MRD-negatieve CR in alle patiënten.

Summary: A study at the Chinese PLA General Hospital
(Beijing) found that bispecific autologous CD19/CD22 CAR-T cell therapy is
feasible and safe and mediates potent anti-leukemic activity in patients with
relapsed or refractory B-ALL. MRD negative complete remission was achieved in 6 out of 6 enrolled patients.
