De Spaanse multicenter
fase3-studie PETHEMA/GEM2012 evalueerde zes cycli
bortezomib-lenalidomide-dexamethason (VRD) als inductietherapie voorafgaand aan
autologe stamceltransplantatie, gevolgd door twee cycli VRD als consolidatie,
voor nieuw-gediagnostiseerd multipel myeloom (NDMM) in patiënten in de leeftijd
tot en met 65 jaar. De studie includeerde 458 patiënten. Dr. Joan Bladé
(Academisch Ziekenhuis, Barcelona) en collega’s publiceren de studie online in Blood.1
De
inductiebehandeling resulteerde in respons die dieper werd in de loop van de
tijd, van tenminste zeer goed partiële respons in 55,6% van de patiënten na
cyclus drie; 63,8% na cyclus vier; 68,3% na cyclus vijf; en 70,4% na cyclus zes
(onder 426 patiënten die cyclus zes voltooiden). Complete respons aan het eind
van de inductiebehandeling werd gezien in 33,4% (34,8% onder patiënten met
hoog-risico genetica). Het percentage patiënten met complete respons nam verder
toe tijdens de vervolgbehandeling, via 44,1% na autoSCT tot 50,2% na
consolidatie. Het percentage patiënten met niet-detecteerbare minimale
residuele ziekte was 28,8% na inductie; 42,1% na transplantatie; en 45,2% na
consolidatie. Graad 3 of hoger adverse
events waren neutropenie (12,9% van de patiënten) en infectie (9,2%).
Perifere neuropathie graad 2 tijdens inductie werd gezien in 17,0%; graad 3 in
3,7%; en graad 4 in één patiënt.
De
onderzoekers concluderen dat VRD een zeer effectief en goed-verdragen
inductieregime is voor NDMM.

tolerated as induction therapy before autoSCT for multiple myeloma, achieving
33.4% complete remission and 28.8% MRD-undetectable rates after six cycles of
induction. Responses deepened throughout induction and over the course of treatment
with few discontinuations due to toxicity.
