Er is geen
duidelijkheid over de waarde van circulerend tumor DNA als
risicostratificerende biomarker in patiënten met Wilms tumor (WT). Een analyse
van resultaten van de Children’s Oncology
Group AREN0533 studie heeft deze waarde geïnventariseerd. Dr. Brian
Crompton (Dana-FarberCancer Institute, Boston MA) en collega’s publiceren de analyse in het Journal of Clinical
Oncology.1
Onder de 395
kinderen met stadium III of IV WT in AREN0533 waren er 50 met opgeslagen
pretreatment serum- en tumormonsters. In 41 van 50 serummonsters werd ctDNA
gedetecteerd. Concordantie tussen serum ctDNA en tumorDNA was 77% voor 1q-gain,
88% voor 16q-deleties, en 70% voor 1p-deleties. Single-nucleotide varianten die
in tumors werden gezien konden ook in ctDNA worden geïdentificeerd. Er was een
trend van slechtere gebeurtenisvrije overleving en overall survival in patiënten met
detecteerbaar ctDNA in pretreatment serummonsters. Alle gevallen van recidief
en overlijden waren in patiënten met detecteerbaar ctDNA in pretreatment
monsters.
De
onderzoekers concluderen dat de analyse suggereert dat ctDNA een bruikbare en
eenvoudig te verkrijgen prognostische biomarker is in patiënten met stadium III
en IV WT.

Summary: Analysis of results of the
Children’s Oncology Group AREN0533 found that ctDNA was detected in the pretreatment
serum of 41 of 50 (82%) of Wilms tumor stage III or IV patients. Detection of
ctDNA was associated with a trend towards worse EFS and OS. All recurrences and
deaths occurred in patients who had detectable levels of ctDNA at the time of
diagnosis.
