Een hoog
percentage patiënten met mammacarcinoom (BC) gebruiken tegelijkertijd adjuvante
hormoontherapie en cardiovasculaire therapie. Een bevolkings-gebaseerde
cohortstudie in Stockholm heeft het relatieve risico geïnventariseerd van
discontinuering van cardiovasculaire therapie in de perioden voor en na
discontinuering van adjuvante hormoontherapie onder BC-patiënten. Prof. Kamila
Czene (Karolinska Instituut) en collega’s publiceren de studie in JAMA Network Open.1
De studie
includeerde alle vrouwen die tussen begin juli 2005 en eind augustus 2020 in de
leeftijd van 40 tot 75 jaar in Stockholm die concomitant adjuvante
hormoontherapie voor BC en cardiovasculaire therapie gebruikten (n=5493). De
mediane follow-up was 7,2 jaar. Het primaire eindpunt van de studie was
discontinuering van de cardiovasculaire therapie tussen één jaar voor en één
jaar na discontinuering van de adjuvante hormoontherapie (‘concomitante
discontinuering’).
Er waren
1811 patiënten die adjuvante hormoontherapie discontinueerden. Deze patiënten
werden 1:1 gematcht voor jaar van BC-diagnose, leeftijd bij diagnose, en
gebruik van dezelfde cardiovasculaire therapie met patiënten die de adjuvante
hormoontherapie voortzetten. Er waren 248 patiënten (12,2%) met concomitante
discontinuering van beide therapieën. In de loop van de follow-up was er een
hoger percentage patiënten met discontinuering van cardiovasculaire therapie
onder patiënten met discontinuering van adjuvante hormoontherapie.
Discontinuering van adjuvante hormoontherapie was geassocieerd met hoger risico van overlijden aan BC (HR 1,43; 95%-bti
1,01-2,01) en aan cardiovasculaire ziekte (1,79; 1,15-2,81).
De
onderzoekers concluderen dat onder BC-patiënten die adjuvante hormoontherapie
discontinueerden de waarschijnlijkheid van eveneens discontinueren van
cardiovasculaire therapie verhoogd was.

Summary: A population-based cohort study in
Stockholm (Sweden) found that breast cancer patients who discontinued adjuvant
hormone therapy were also more likely to discontinue cardiovascular therapy.
The authors conclude that clinicians should switch from single- to
multiple-disease focus to prevent discontinuation of therapies for other
diseases among patients with breast cancer.
