Patiënten met N2-3 nasofarynxcarcinoom (NPC) hebben een hoog risico van falen van de behandeling ondanks gebruik van een concurrent adjuvant cisplatine-fluorouracil regime. Een multicenter fase 3-studie in China heeft cisplatine-gemcitabine in deze setting vergeleken met cisplatine-fluorouracil. Resultaten van de studie zijn gisteren gepresenteerd op de Annual Meeting van ASCO in Chicago. Prof. Hai-Qiang Mai (Sun Yat-sen Universiteit, Guangzhou) publiceren de studie ook in The Lancet Oncology.¹

De studie includeerde patiënten (leeftijd 18 tot en met 65 jaar) met niet-eerder behandeld stadium T1-4 N2-3 M0 NPC, een ECOG performance status 0 of 1, en adequate orgaanfunctie. De patiënten kregen concurrent cisplatine plus IMRT, gevolgd door cisplatine-gemcitabine (n=120) of cisplatine-fluorouracil (n=120). Het primaire eindpunt was drie-jaars progressievrije-overlevingspercentage.

Op het moment van data cutoff voor de nu gepresenteerde analyse was de mediane follow-up 40 maanden (IQR 32-48). Het drie-jaars PFS-percentage was 83,9% in de cisplatine-gemcitabinegroep versus 71,5% in de cisplatine-fluorouracilgroep (sHR 0,54; p=0,023). De meest-gerapporteerde graad 3 of hoger adverse events tijdens de behandeling waren leukopenie (52% in de cisplatine-gemcitabinegroep versus 29% in de cisplatine-fluorouracilgroep, neutropenie (32% versus 16%), en mucositis (23% versus 28%). Drie maanden of langer na de behandeling werd gehoorverlies gezien in 5% versus 9%. Eén patiënt in de cisplatine-gemcitabinegroep overleed aan complicaties van de behandeling.

De onderzoekers concluderen dat concurrent adjuvant cisplatine-gemcitabine kan worden gebruikt als adjuvante therapie voor N2-3 NPC.
