Consolidatie-radiotherapie naar bulky lesies van gevorderd stadium Hodgkin lymfoom na complete respons op ABVD


Prof. Umberto RicardiOngeveer 65% van de patiënten met gevorderd stadium Hodgkin lymfoom heeft complete respons na zes cycli ABVD-chemotherapie. Er is geen consensus over de rol van consoliderende radiotherapie na de chemotherapie. De Italiaanse fase 3-studie HD0801 randomiseerde patiënten met complete respons na chemotherapie naar al of niet radiotherapie. Prof. Umberto Ricardi (Universiteit van Turijn) presenteert de studie vandaag op het ESTRO 38 congres in Milaan.1

De studie includeerde 512 patiënten met gevorderd stadium HL die zes cycli ABVD kregen. PET-bevestigde complete respons werd gezien in 354 patiënten, van wie 116 met bulky lesies bij baseline (groter dan 5 cm diameter). Deze patiënten werden gerandomiseerd naar radiotherapie (RT) of no further treatment (NFT). De progressievrije overleving na drie jaar was 91,7% in de RT-groep versus 81,4% in de NFT-groep, en de PFS na vijf jaar was 88,9% in de RT-groep versus 81,5% in de NFT-groep. Deze verschillen zijn niet statistisch significant, volgens de onderzoekers vanwege een te gering aantal patiënten en gebeurtenissen.

De onderzoekers concluderen dat patiënten met gevorderd HL na complete respons of ABVD baat kunnen hebben bij radiotherapie naar baseline bulky lesies, met een PFS-profijt van ongeveer 10% na drie jaar en 7% na vijf jaar.
