De
combinatie van oxaliplatine en fluoropyrimidine gedurende zes maanden is een
standaard-optie voor adjuvante therapie voor hoog-risico stadium II of stadium
III colorectaalcarcinoom. Een van de met oxaliplatine samenhangende
bijwerkingen is neurotoxiciteit. De multicenter gerandomiseerde noninferioriteits
fase 3-studie KCSG CO09-07 in Zuid-Korea heeft drie versus zes maanden
oxaliplatine in combinatie met zes maanden fluoropyrimidine voor hoog-risico
stadium II of stadium III CRC geëvalueerd. Prof. Young Suk Park (Samsung
Medisch Centrum, Seoel) en collega’s publiceren de studie in het Journal of Clinical
Oncology.1
De studie
includeerde 1788 patiënten, die werden gerandomiseerd naar zes maanden
oxaliplatine (zes-maands arm; n=895) of drie maanden oxaliplatine (drie-maands
arm; n=893) toegevoegd aan zes maanden fluoropyrimide. Het primaire eindpunt
was drie-jaars ziektevrije overleving, met een noninferioriteitsmarge HR 1,25.
In de zes-maands arm voltooiden 83,6% van de patiënten de behandeling, en in de
drie-maands arm 85,7%. Any grade
neuropathiepercentages waren hoger in de zes-maands arm dan in de drie-maands
arm (69,5% versus 58,3%; p<0,0001). De drie-jaars DFS-percentages waren 83,7% in de zes-maands arm versus 84,7% in de
drie-maands arm (p voor noninferioriteit 0,0065). Onder patiënten met stadium
III CRC die capecitabine plus oxaliplatine kregen was de drie-jaars DFS van de
drie-maands arm niet-inferieur aan die van de zes-maands arm (HR 0,713;
p=0,0009). Echter, onder patiënten met hoog-risico stadium II en stadium III
CRC die fluorouracil, leucovorine, en oxaliplatine kregen werd de
noninferioriteit in de drie-maands arm versus de zes-maands arm niet bewezen.
De
onderzoekers concluderen dat toevoegen van drie maanden oxaliplatine aan zes
maanden capecitabine een alternatieve adjuvante behandeling voor stadium III
hoog-risico CRC kan zijn, met minder neurotoxiciteit dan met zes maanden
oxaliplatine.

randomized phase 3 KCSG CO09-07 trial in South Korea found that adding 3 months of oxaliplatin to 6 months of
capecitabine resulted in noninferior three-year DFS compared to adding 6 months
of oxaliplatin to 6 months of capecitabine for high-risk stage III CRC, with
lower neuropathy rates.
