Platina-gebaseerde
chemotherapie heeft slechts bescheiden werkzaamheid voor gevorderd
niet-kleincellig longcarcinoom, maar kan wellicht immuunresponsen versterken.
De fase 1/2-studie KEYNOTE-021 heeft de werkzaamheid en veiligheid onderzocht
van van platina-doublet chemotherapie in combinatie met de
immuuncheckpointremmer pembrolizumab als eerstelijns behandeling voor gevorderd
NSCLC. Dr. Shirish Gadgeel (Wayne State University, Detroit MI) en collega’s publiceren
de studie online in Lung Cancer.1
De studie
includeerde 74 patiënten, met niet-eerder behandeld gevorderd NSCLC zonder EGFR/ALK-veranderingen, in drie cohorten.
Patiënten in cohort A (any histology)
kregen pembrolizumab 2 of 10 mg/kg iedere drie weken plus carboplatine AUC 6
mg/ml/min plus paclitaxel 200 mg/m2. Patiënten in cohort B
(niet-squameus NSCLC) kregen pembrolizumab plus carboplatine AUC 6 mg/ml/min
plus paclitaxel 200 mg/m2 plus bevacizumab 15 mg/kg. Patiënten in
cohort C (niet-squameus NSCLC) kregen pembrolizumab plus carboplatine AUC 5
mg/ml/min plus pemetrexed 500 mg/m2. Na vier cycli kregen de
patiënten onderhoudsbehandeling met pembrolizumab (cohort A), pembrolizumab
plus bevacizumab (cohort B), of pembrolizumab plus pemetrexed (cohort C).
De mediane
follw-up was 21,4 maanden in cohort A; 16,4 maanden in cohort B; en 17,4
maanden in cohort C. In geen van de cohorten werden pembrolizumab-doseringslimiterende
toxiciteiten gezien. De meest-frequente behandelings-gerelateerde adverse events waren alopecie,
vermoeidheid, en misselijkheid. Graad 3 of 4 AEs werden gezien in 40% van de
patiënten in cohort A, 42% in cohort B, en 46% in cohort C. De geblindeerd
centraal-beoordeelde ORRs in de drie cohorten waren 48%, 56%, en 75%.
De
onderzoekers concluderen dat pembrolizumab in combinatie met
carboplatine-paclitaxel of pemetrexed-carboplatine voor gevorderd NSCLC
resulteerde in bemoedigende antitumor-activiteit met toxiciteit die consistent
was met bekende toxiciteit van pembrolizumab monotherapie en platina-gebaseerde
chemotherapie.

Summary: The
KEYNOTE-021 study showed that in
patients with previously untreated advanced NSCLC without EGFR/ALK aberrations pembrolizumab in combination with
carboplatin-paclitaxel or pemetrexed-carboplatin yielded encouraging antitumor
activity and toxicity consistent with known toxicity of pembrolizumab
monotherapy or platinum-based chemotherapy.
