Postoperatieve
ileus (POI) vertraagt het herstel na colorectale resectie. Een gerandomiseerde
multicenterstudie in China heeft werkzaamheid van electroacupunctuur (EA) voor
verkorting van POI geëvalueerd. Prof. Cun-Zhi Liu (Universiteit voor Chinese
Geneeskunde, Beijing) en collega’s publiceren de studie in JAMA Surgery.1
De studie
includeerde 249 vowassen patiënten die na laparoscopische resectie volgens het Enhanced Recovery After Surgery
(ERAS)-protocol 1:1 werden gerandomiseerd naar vier EA-sessies of vier sham sessies (SA). Het primaire eindpunt
van de studie was de tijd tot eerste defecatie. Deze bedroeg mediaan 76,4 uur in de EA-groep versus 90,0 uur in de SA-groep
(p=0,003). In de EA-groep vergeleken met de SA-groep waren ook de resultaten
beter voor de secundaire eindpunten tijd tot eerste flatus (mediaan 44,3 versus
58,9 uur; p<0,001), tijd tot verdragen van semiliquide voeding ( mediaan 105,8
versus 116,5 uur; p=0,01), en tijd tot verdragen van vaste voeding (mediaan 181,8
versus 190,3 uur; p=0,01). EA was niet geassocieerd met ernstige adverse events.
De
onderzoekers concluderen dat onder patiënten die laparoscopische chirurgie voor
CRC ondergingen, EA resulteerde in kortere duur van POI. EA kan worden gezien
als ondersteuning van het ERAS-protocol om het herstel van de gastroïntestinale
functie te bevorderen.
