De
multinationale fase 3-studie PALOMA-3 randomiseerde vrouwen met HR-positief
HER2-negatief gevorderd mammacarcinoom (ABC) 2:1 naar palbociclib plus
fulvestrant (palbociclibgroep; n=347) of placebo plus fulvestrant (placebogroep;
n=174). In 2015 is gepubliceerd dat de mediane progressievrije overleving 9,2 maanden bedroeg in
de palbociclibgroep versus 3,8 maanden in de placebogroep (HR 0,42;
p<0,001). Prof. Massimo Cristofanilli (Weill Cornell Medicine, New York) en
collega’s publiceren nu in Clinical
Cancer Research een update van de studie met mediaan 73,3 maanden follow-up.1
De nu
gepubliceerde analyse is uitgevoerd nadat 393 van de 521 gerandomiseerde
patiënten overleden waren (75,4%). De mediane overall survival was 34,8 maanden in de palbociclibgroep versus
28,0 maanden in de placebogroep (HR 0,81; 95%-bti 0,65-0,99). Het zes-jaars
OS-percentage was 19,1% in de palbociclibgroep versus 12,9% in de placebogroep.
Gunstige OS in de palbociclibgroep versus de placebogroep werd gezien in de
meeste subgroepen, waaronder patiënten met endocriengevoelige ziekte, geen
eerdere chemotherapie voor ABC, lage circulerende tumorfractie, en ongeacht ESR1-, PIK3CA-, of TP53-mutatiestatus.
Er waren geen nieuwe veiligheidssignalen.
De
onderzoekers concluderen dat toevoeging van palbociclib aan fulvestrant voor
HR-positief HER2-negatief ABC resulteerde in klinisch relevante verbetering van
de OS die langer dan zes jaar aanhield.

Summary: Updated analyses of PALOMA-3 (palbociclib plus fulvestrant versus placebo plus
fulvestrant for HR+/HER2- advanced breast cancer) found that clinically
meaningul improvement in overall survival in the palbociclib group was
maintained with longer than 6 years of follow-up.
