Lymphocyte-activation gene 3 (LAG-3) is een immuunreceptor die activering van T-cellen remt. Ieramilimab is een remmer van LAG-3. Een multinationale fase 1-2 studie heeft ieramilimab met of zonder het anti-PD-1 antilichaam spartalizumab geëvalueerd voor de behandeling van gevorderde maligniteiten. Dr. David Hong (MD Anderson Cancer Center, Houston TX) en collega’s publiceren de studie in het Journal for ImmunoTherapy of Cancer.¹


De studie includeerde patiënten met gevorderde of metastatische solide tumoren, met progressie na of intolerantie tegen standaard-behandelingen. Er waren 134 patiënten die ieramilimab monotherapie kregen en 121 patiënten die ieramilimab plus spartalizumab combinatietherapie kregen. In de doseringsescalatiefase werd geen maximaal-verdragen dosering geïdentificeerd. Als aanbevolen fase 2-dosering werd gekozen voor 400 mg ieramilimab plus 300 mg spartalizumab iedere drie weken, of 800 mg ieramilimab plus 400 mg spartalizumab iedere vier weken. Treatment-related adverse events (vooral graad 1 of 2) werden gezien in 56% van de patiënten in de monotherapiegroep en 69% van de patiënten in de combinatietherapiegroep. Antitumoractiviteit werd gezien in de combinatietherapiegroep, met complete respons in 2% en partiële respons in 8% van de patiënten, en stabiele ziekte gedurende tenminste zes maanden in nog 6,6% van de patiënten.

De onderzoekers concluderen dat ieramilimab zowel als monotherapie als in combinatie met spartalizumab goed verdragen werd. De combinatie resulteerde in bescheiden antitumoractiveit in patiënten met eerder-behandelde gevorderde solide maligniteiten.

Summary: A multinational phase 1-2 study found that the LAG-3 inhibitor ieramilimab for advanced malignancies was well tolerated as monotherapy and in combination with spartalizumab (anti-PD-1). The toxicity of the combination was comparable to that of spartaluzimab alone. Modest antitumor activity was seen with the combination (ORR 10%; SD for 6 months or longer in 6.6%).
