Er is
behoefte aan nieuwe behandelingen voor gevorderd melanoom met progressie op
anti-PD-1 of anti-PD-L1 therapie. Deel D van de multinationale fase 1-2 RELATIVITY-020
studie heeft de combinatie van nivolumab (anti-PD-1) en relatlimab (anti-LAG-3)
in deze setting geëvalueerd. Prof. Paolo Ascierto (Istituto Nazionale Tumori,
Napels) en collega’s publiceren de studie in het Journal of Clinical
Oncology.1
De studie
includeerde patiënten met progressie tijdens of binnen drie maanden na één
(cohort D1; n=354) of meer (D2; n=164) lijnen anti-PD-(L1) omvattende
behandelingen. Coprimaire eindpunten waren centraal onafhankelijke beoordeelde respons
en veiligheid. Onder de voor respons evalueerbare patiënten was de objective response rate 12,0% (95%-bti
8,8-15,8) in D1 en 9,2% (5,2-14,7). Reponsen werden gezien in patiënten met
tumoren met en zonder expressie van PD-L1 en LAG-3. De mediane duur van respons
werd niet bereikt in D1 (95%-bti 12,9-NR) en was 12,8 maanden (6,9-12,9) in D2.
De centraal onafhankelijk beoordeelde mediane PFS was 2,1 maanden (95%-bti 1,9-3,5)
in D1 en 3,2 maanden (1,9-3,6) in D2. De incidentie van graad 3 of 4 treatment-related adverse events was
15,0% in D1 en 12,8% in D2. Er waren geen graad 5 TRAEs.
De
onderzoekers concluderen dat de combinatie van nivolumab en relatlimab een manageable veiligheidsprofiel had en
resulteerde in duurzame klinische actitiveit voor gevorderd melanoom met
progressie op anti-PD-(L)1 therapie.

Summary: The
multinational phase 1-2a RELATIVITY-020 trial evaluated the combination of nivolumab
and relatlimab for advanced melanoma after progression on anti-PD-1 or
anti-PD-L1 therapy. The safety profile was manageable, with no
treatment-related deaths. Objective response rate by blinded independent
central review ranged from 9.2% to 12.0% across study cohorts. Responses were
durable.
