LY3127804 is
een nieuw selectief monoklonaal antilichaam tegen angiopoïetine-2. Een multinationale
fase 1-studie heeft de veiligheid en werkzaamheid van LY3127804 met of zonder
de VEGFR2-remmer ramucirumab geïnventariseerd in patiënten met gevorderde
maligniteiten. Dr. Juan Martin-Liberal (Vall d’Hebron Institut d’ Oncologia,
Barcelona) en collega’s publiceren de studie in het British Journal of Cancer.1
De studie
includeerde patiënten in drie cohorten. Patiënten in cohort A (n=20) kregen
oplopende dosering van LY3127804 monotherapie, van 4 tot 27 mg/kg iedere twee
weken. Patiënten in cohort B (n=35) kregen oplopende doseringen LY3127804, van
8 tot 27 mg/kg, plus ramucirumab 8 mg/kg iedere twee weken. Patiënten in cohort
C kregen LY3127804 20 mg/kg plus ramucirumab 12 mg/kg iedere twee weken.
Doserings-escalatie vond plaats op basis van uitblijven van
doseringslimiterende toxiciteiten in de eerste vier weken van elke dosering.
Er werden
geen DLTs gezien. Als aanbevolen fase 2-dosering werd gekozen voor LY3127804 20
mg/kg iedere twee weken. De meest-gerapporteerde treatment-emergent adverse events waren constipatie, diarree en
vermoeidheid in cohort A, en hypertensie en perifeer oedeem in cohorten B en C.
Partiële respons werd gezien in 4 patiënten met combinatietherapie
(heldercellig endometriumcarcinoom, cervix squameus celcarcinoom, carcinoom van
onbekende primaire origine, en maag-slokdarmovergang carcinoom). Stabiele
ziekte werd gezien in 29 patiënten.
De
onderzoekers concluderen dat LY3127804 als monotherapie en in combinatie met
ramucirumab goed verdragen werd.

Summary: A multinational phase 1 study found
that the anti-angiopoietin-2 monoclonal antibody LY3127804, as monotherapy and
in combination with ramucirumab, was well tolerated by patients with advanced
solid tumors. Partial responses were seen with the combination therapy in clear
cell endometrial carcinoma, cervix squamous cell carcinoma, carcinoma of
unknown primary, and gastroesophageal junction carcinoma. More than half of
patients had stable disease.
