In patiënten
met AML bevordert interleukine-6 (IL-6) chemoresistentie en zijn hogere niveaus
van IL-6 geassocieerd met slechtere prognose. Tocilizumab is een op de
IL6-receptor gericht monoklonaal antilichaam. De fase 1-studie TOCILAM in het Academisch
Ziekenhuis van de Universiteit van Nantes (Frankrijk) heeft tocilizumab in
combinatie met standaard intensieve inductiechemotherapie voor AML geëvalueerd.

De studie
includeerde twaalf volwassen AML-patiënten, onder wie negen met hoog-risico
ziekte, en een ECOG performance status 2 of beter. De patiënten kregen
intraveneus tocilizumab (4, 6, of 8 mg/kg) toegediend op dag acht van een
standaard inductieregime (idarubicine-cytarabine). Er werd geen
doseringlimiterende toxiciteit gezien. Er waren negen ernstige adverse events, allemaal
niet-samenhangend met tocilizumab. Twee patiënten overleden tijdens de
inductiechemotherapie. Onder de overigen tien waren er negen met respons. De overlevingsuitkomsten waren bemoedigend.
De
onderzoekers concluderen dat de combinatie van tocilizumab met standaard
AML-inductiechemotherapie veilig was en resulteerde in bemoedigende responsen.
