Er is geen
duidelijkheid over de optimale behandeling voor kleincellig longcarcinoom na
progressie op twee achtereenvolgende lijnen chemotherapie. Een gerandomiseerde
multicenter fase 2-studie in China heeft anlotinib in deze setting geëvalueerd.
Dr. Jing Cheng (Jilin Maligniteitenziekenhuis, Changchun) en collega’s publiceren
de studie in het British Journal of Cancer.1
De studie,
uitgevoerd in elf centra, includeerde patiënten met pathologisch bevestigd SCLC
na falen van tenminste twee lijnen chemohterapie. De patiënten werden 2:1
gerandomiseerd naar oraal anlotinib 12 mg eenmaal daags op de eerste veertien
dagen van drie-weekse cycli (n=82) of placebo (n=38). Het primaire eindpunt was
progressievrije overleving. De mediane PFS was 4,1 maanden met anlotinib versus
0,7 maanden met placebo HR 0,19; p<0,0001) en de mediane overall survival was 7,3 maanden met
anlotinib versus 4,9 maanden met placebo (HR 0,53; p=0,0029). Het
veiligheidsprofiel van anlotinib was gunstig.
De
onderzoekers concluderen dat anlotinib activiteit had als derde of latere lijn
behandeling voor SCLC na progressie op twee lijnen chemotherapie.
