Er zijn
weinig behandelopties voor patiënten met hoog-risico chemorefractair of
recidiverend gestationale trofoblastaire neoplasie (GTN). In veel typen solide
tumoren is synergie gezien van immuuntherapie en antiangiogenese. De fase
2-studie CAP 01 in Beijing heeft de combinatie van camrelizumab en apatinib
voor hoog-risico chemorefractair of recidiverend GTN geëvalueerd. Prof. Yang
Xiang (Chinese Academie van Medische Wetenschappen) en collega’s publiceren de studie in The Lancet Oncology.1
De studie
includeerde vrouwen in de leeftijd van achttien tot en met zeventig jaar met
hoog-risico (FIGO score 7 of hoger) GTN die tenminste twee eerdere lijnen
multichemotherapie hadden gekregen, en een ECOG performance status 2 of beter
hadden. De patiënten kregen intraveneus camrelizumab 200 mg iedere twee weken
plus oraal apatinib 250 mg eenmaal daags. De behandeling werd voortgezet tot ziekteprogressie
of niet-acceptabele toxiciteit. Het primaire eindpunt was percentage patiënten
met objectieve respons, bepaald aan de hand van de serum HCG-concentratie.
De studie
includeerde twintig patiënten, onder wie negentien met choriocarcinoom en één
met placental site trophoblastic tumor
(PSTT). De mediane duur van follow-up was 18,5 maanden (IQR 14,6-20,9).
Objectieve respons werd gezien in elf patiënten (ORR 55%; 95%-bti 32-77) en
complete respons in tien patiënten (50%; 95%-bti 27-73). De meest-waargenomen graad 3 treatment-related
adverse events waren hypertensie (25% van de patiënten), rash (20%),
neutropenie (10%), leukocytopenie (10%), en aspartaataminotransferase-toename
(10%). Er waren geen graad 4 of 5 TRAEs.
De
onderzoekers concluderen dat onder patiënten met hoog-risico chemorefractair of
recidiverend GTN de combinatie camrelizumab plus apatinib veelbelovende
antitumor-activiteit en acceptabele toxiciteit had.
