Inductiebehandeling
met ibrutinib plus rituximab biedt de mogelijkheid om blootstelling van
patiënten met mantelcellymfoom (MCL) aan chemotherapie te minimaliseren omdat
de inductiebehandeling kan resulteren in remissie zonder chemotherapie en
wellicht consolidatie met vier cycli chemotherapie in plaats van de
conventionele acht cycli mogelijk kan maken. De fase 2-studie WINDOW-1 van MD
Anderson Cancer Center (Houston TX) heeft inductie met ibrutinib-rituximab
gevolgd door vier cycli R-HCVAD (rituximab plus hypergefractioneerd
cyclofosfamide, vincristine, doxorubicine, en dexamethason) afwisselend met
methotrexaat-cytarabine geëvalueerd voor niet-eerder behandeld MCL in patiënten
jonger dan 66 jaar. Prof. Michael Wang en collega’s publiceren de studie in The Lancet Oncology.1
De studie
includeerde 131 patiënten (mediane leeftijd 56 jaar; IQR 49-60) met een ECOG
performance status 2 of beter. In de inductiefase (deel A van de studie) kregen
de patiënten twaalf cycli ibrutinib-rituximab. Zodra patiënten complete respons
hadden gingen ze door naar deel B, bestaande uit vier cycli R-HCVAD afgewisseld
met methotrexaat-cytarabine. Patiënten die na deel A geen complete respons
hadden kregen twee cycli R-HCVAD afgewisseld met methotrexaat-cytarabine
gevolgd door herbeoordeling, tot ten hoogste acht cycli. Patiënten met stabiele
ziekte of progressie tijdens R-HCVAD werden uit de studie genomen.
Het primaire
eindpunt was overall respons na deel A. Deze werd gezien in 129 van 131
patiënten (98%; 95%-bti 95-100). De meest-waargenomen graad 3 of 4 adverse
events waren lymfocytopenie (14% van de patiënten), rash (12%), tromboytopenie
(9%), infecties (8%), en vermoeidheid (8%) in deel A, en lymfocytopenie (73%),
leukocytopenie (32%), trombocytopenie (30%), en neutropenie (20%) in deel B.
Eén patiënt overleed, aan een als niet-studiegerelateerd beschouwde oorzaak.
De
onderzoekers concluderen dat inductie met ibrutinib-rituximab in de frontline
behandeling van MCL in jonge patiënten actief en veilig was, en vermindering
van de blootstelling aan chemotherapie mogelijk maakte.
