De
PARP-remmer talazoparib is actief voor gevorderd mammacarcinoom (BC) met kiemlijnmutaties
in BRCA1/2 (gBRCA1/2). De multicenter fase 2-studie Talazoparib Beyond BRCA in
de Verenigde Staten heeft talazoparib monotherapie voor maligniteiten in
patiënten met wildtype BRCA1/2 maar
mutaties in andere genen die betrokken zijn bij de homologe-recombinatie (HR)-route
geëvalueerd. Dr. Joshua Gruber
(University of Texas Southwestern Medical Center, Dallas) en collega’s
publiceren de studie in Nature Cancer.1
De studie
includeerde patiënten met eerder-behandeld gevorderd HER2-negatief BC (n=13) of
andere solide tumoren (n=7) met mutaties in andere HR-routegenen dan BRCA1 en BRCA2. Talazoparib induceerde objectieve respons in vier van de BC-patiënten (ORR 31%), en stabiele
ziekte langer dan zes maanden in drie andere patiënten (CBR 54%). Alle
patiënten met kiemlijnmutaties in PALB2 (gPALB2) hadden behandelings-gerelateerde
tumorregressie. De HR-deficiëntiescores van tumor-DNA of plasma circulerend
tumor DNA waren geassocieerd met uitkomsten van de behandeling. Een gPALB2-geassocieerd mutatiesignatuur was
geassocieerd met de respons op talazoparib.
De
onderzoekers concluderen dat talazoparib werkzaamheid had in patiënen met
gevorderd mammacarcinoom en gPALB2-mutaties.
