Neoadjuvante chemotherapie (NACT) is standaard voor stadium II of III triple-negatief mammacarcinoom (TNBC). De gerandomiseerde fase 2-studie NACATRINE in het Hospital de Câncer de Barretos (Brazilië) heeft toevoeging van carboplatine aan standaard NACT voor stadium II of III TNBC geëvalueerd. De studie includeerde 146 patiënten met stadium II of III TNBC. Alle patiënten kregen neoadjuvant vier drie-wekelijkse cycli doxorubicine plus cyclofosfamide, en werden vervolgens gerandomiseerd naar twaalf wekelijkse cycli paclitaxel met (n=73) of zonder (n=73) wekelijks carboplatine AUC 1,5. De randomisatie geschiedde gestratificeerd naar gBRCA status, leeftijd, en stadium (II versus III). Het primaire eindpunt was percentage patiënten met pathologisch complete respons. Dit percentage bedroeg 43,8% in de groep met carboplatine versus 30,1% in de groep zonder carboplatine. Met deze verbetering met 13,7% werd niet voldaan aan de vooraf-gespecificeerde doelstelling van verbetering van de pCR met 15%. De patiënten worden gevolgd voor overlevingsuitkomsten.

De onderzoekers concluderen dat toevoegen van carboplatine aan standaard NACT voor stadium II of III TNBC resulteerde in numerieke verbetering van het pCR-percentage.

Summary: The randomized phase 2 NACATRINE trial at Barretos Cancer Hospital (Brazil) found that the addition of carboplatin to standard neoadjuvant chemotherapy for stages II and III triple-negative breast cancer was associated with a numerical increase in the pCR rate. Follow-up for survival outcomes is ongoing.
