Fase 3-studie van eerstelijns cemiplimab monotherapie voor gevorderd NSCLC met PD-L1 expressie tenminste 50%


Prof. Ahmet SezerCemiplimab is een nieuwe PD-1 remmer. De multinationale fase 3-studie EMPOWER-Lung 1 evalueerde cemiplimab voor niet-eerder behandeld gevorderd kleinceliig longcarcinoom (aNSCLC). Prof. Ahmet Sezer (Başkent Universiteit, Adana Turkije) en collega’s publiceren de studie in The Lancet.1

EMPOWER-Lung 1 werd uitgevoerd in 138 centra in 24 landen. De studie includeerde volwassen patiënten met aNSCLC, ECOG performance status 0 of 1, met geschiedenis van roken (of huidige rokers). De patiënten werden 1:1 gerandomiseerd naar cemiplimab 350 mg iedere drie weken of platina-doublet chemotherapie. Crossover van chemotherapie naar cemiplimab was toegestaan na progressie. Primaire eindpunten waren overall survival en progressievrije overleving in alle patiënten en in groep patiënten met tumoren met tenminste 50% PD-L1 expressie.
De studie includeerde 710 patiënten, onder wie 563 met PD-L1 tenminste 50%. In deze laatste groep werd de mediane OS niet bereikt met cemiplimab (n=283) en was de mediane OS 14,2 maanden met chemotherapie (n=280; HR 0,57; p=0,0002). De mediane PFS in de PD-L1 tenminste 50% groep was 8,2 maanden met cemiplimab versus 5,7 maanden met chemotherapie (HR 0,54; p<0,0001). Ook onder alle patiënten was de OS significant beter met cemiplimab dan met chemotherapie ondanks een hoog crossover percentage (74%). Graad 3 of 4 treatment-related adverse events werden gezien in 28% van de cemiplimab-patiënten en 39% van de chemotherapie-patiënten.

De onderzoekers concluderen dat eerstelijns cemiplimab monotherapie vergeleken met chemotherapie voor aNSCLC met PD-L1 expressie tenminste 50% resulteerde in significant betere OS en PFS.

Summary: The multinational phase 3 study EMPOWER-Lung 1 found that first-line cemiplimab monotherapy compared with chemotherapy significantly improved progression-free and overall survival in patients with advanced non-small-cell lung cancer with PD-L1 of at least 50%, providing a new treatment option for this patient population.
