Standaard
chemotherapie voor squameus niet-kleincellig longcarcinoom (sqNSCLC) is platina
plus gemcitabine (GP). Het anti-PD-1 antilichaam sintilimab in combinatie met
GP had in een fase 1b-studie bemoedigende activiteit als eerstelijns
behandeling voor sqNSCLC. De Chinese multicenter fase 3-studie ORIENT-12 heeft
deze combinatie geëvalueerd als eerstelijns behandeling voor lokaal-gevorderd of
metastatisch sqNSCLC zonder EGFR- of ALK-veranderingen. Prof. Caicun Zhou
(Shanghai Pulmonair Ziekenhuis) en collega’s publiceren de studie in het Journal of Thoracic
Oncology.1
ORIENT-12
werd uitgevoerd in 42 centra. De studie includeerde 357 patiënten, die 1:1 werden
gerandomiseerd naar vier of zes cycli sintilimab 200 mg plus GP iedere drie
weken (sintilimabgroep; n=179) of placebo plus GP iedere drie weken
(placebogroep; n=178), gevolgd door sintilimab- of placebo-onderhoudstherapie
tot ziekteprogessie, gedurende ten hoogste twee jaar. De randomisatie geschiedde
gestratificeerd naar klinisch stadium, type platinachemotherapie, en PD-L1 tumor proportion score (TPS). Het
primair eindpunt was centraal-beoordeelde progressievrije overleving.
De mediane
follow-up was 12,9 maanden. De PFS was significant langer in de sintilimabgroep
dan in de placebogroep (HR0,536; p<0,00001). De zes-maands PFS was 41,4% (95%-bti
32,8-49,8) in de sintilimabgroep versus 24,7% (95%-bti 17,8-32,2) in de
placebogroep. Graad 3 of hoger treatment-emergent
adverse events werden gezien in 86,6% van de patiënten in de
sintilimabgroep versus 83,1% van de placebogroep, en graad 5 TEAEs in 4,5%
versus 6,7%.
De
onderzoekers concluderen dat toevoegen van sintilimab aan eerstelijns GP voor
gevorderd sqNSCLC resulteerde in significante verlenging van de PFS. De
toxiciteit was acceptabel, en er waren geen nieuwe veiligheidssignalen.

Chinese multicenter phase 3 ORIENT-12 study found that addition of sintilimab
to first-line platinum and gemcitabine for advanced squamous NSCLC resulted in
significant PFS benefit (HR 0.536; p=0.00001). The toxicity was acceptable.
