Profylactische
bestraling van de borstwand na diagnostische of therapeutische procedures in
patiënten met maligne pleuraal mesothelioom (MPM) wordt in Europa veel
toegepast, hoewel de werkzaamheid van deze behandeling onzeker is. Een
multicenter fase 3-studie in 54 centra in Groot-Brittannië heeft de waarde van
deze bestraling voor de preventie van borstwandmetatasen (CWMs) onderzocht. Dr.
Corinne Faivre-Finn (University of Manchester) en collega’s publiceren de studie online in het Journal of Clinical
Oncology.1
De studie
includeerde 375 MPM patiënten die werden gerandomiseerd naar profylactische
bestraling (n=186) of geen bestraling (n=189) binnen 42 dagen na het ondergaan
van een borstwandprocedure. Profylactische bestraling was 21 Gy in drie
fracties op drie achtereenvolgende werkdagen. Het primaire eindpunt van de
studie was incidentie van CWMs binnen zes maanden na randomisatie. CWMs werden
gezien in zes patiënten in de bestraalde groep (3,2%) versus tien patiënten in
de controlegroep (5,3%; OR 0,60; p=0,44). Stralingsdermatitis graad 1 werd
gezien in 51,6% van de bestraalde patiënten, graad 2 in 10,2%, en graad 3 in
één patiënt.
De
onderzoekers concluderen dat er geen rol is voor routinematig gebruik van
propfylactische bestraling naar sites
van borstwandprocedures in MPM-patiënten.
