Het
anti-PD-l1 antilichaam atezolizumab verbetert de overall survival van patiënten met eerder-behandeld NSCLC. De
multinationale fase 3-studie IMpower130 heeft de waarde onderzocht van toevoeging
van atezolizumab aan eerstelijns chemotherapie voor metastatisch niet-squameus
NSCLC. Dr. Howard West (Swedish
Cancer Institute, Seattle WA) en collega’s publiceren de studie online in The Lancet Oncology.1
IMpower130
werd uitgevoerd in 131 centra in acht landen. De studie includeerde 723
patiënten met stadium IV nsq NSCLC, onder wie 679 met EGFR- en ALK-wildtype
tumoren. De patiënten werden 2:1 gerandomiseerd naar intraveneus atezolizumab
1200 mg eens per drie weken) plus carboplatine en nab-paclitaxel chemotherapie
(n=483 onder wie 451 met wildtype tumoren) of alleen carboplatin en
nab-paclitaxel chemotherapie (n=240 onder wie 228 met wildtype tumoren). Primaire
eindpunten van de studie waren de door lokale onderzoekers beoordeelde
progressievrije overleving en de overall
survival, beide in de groep patiënten met wildtype tumoren.
Er was geen
significant verschil tussen beide armen in mediane follow-up: 18,5 maanden in
de atezolizumab-plus-chemotherapie arm versus 19,2 maanden in de
alleen-chemotherapie arm. De mediane OS in de wildtype-populatie was 18,6
maanden in de atezolizumab-plus-chemotherapie arm versus 13,9 maanden in de
alleen-chemotherapie arm (HR 0,79; p=0,033). De mediane PFS in de
wildtype-populatie was 7,0 maanden in de atezolizumab-plus-chemotherapie arm
versus 5,5 maanden in de alleen-chemotherapie arm (HR 0,64; p<0,0001). Er
waren geen nieuwe veiligheidssignalen. Behandelingsgerelateerde ernstige adverse events werden gerapporteerd voor
24% van de patiënten in de atezolizumab-plus chemotherapie arm versus 13% van
de patiënten in de alleen-chemotherapie arm. Graad 5 AEs troffen acht patiënten
(2%) in de atezolizumab-plus-chemotherapie-arm versus één patiënt (<1%) in
de alleen-chemotherapie arm.
De onderzoekers
concluderen dat toevoegen van atezolizumab aan eerstelijns chemotherapie
resulteerde in significant langere PFS en OS in patiënten met mnsq NSCLC zonder
mutaties in ALK of EGFR. De studie identificeerde geen
nieuwe veiligheidsignalen.

Summary: The
international phase 3 study IMpower130 compared atezolizumab in combination with carboplatin plus nab-paclitaxel
chemotherapy versus chemotherapy alone as first-line treatment for metastatic
non-squamous NSCLC without ALK or EGFR mutations. The study showed a
significant and clinically meaningful improvement in overall survival and a
significant improvement in progression-free survival in the atezolizumab arm.
There were no new safety signals.
