Topotecan is
in Europa de enige toegestane behandeling voor kleincellig longcarcinoom in de
tweedelijns setting. Een multicenter fase 3-studie in Frankrijk heeft de
doublet carboplatine-etoposide vergeleken met topotecan als tweedelijns
behandeling voor SCLC met aanvankelijke respons op eerstelijns
platina-etoposide, maar met progressie tenminste negentig dagen na voltooiing
van de eerstelijns behandeling. Prof.
Christos Chouaid (Centre Hospital Intercommunal de Créteil) en collega's publiceren
de studie in The Lancet Oncology.1
De studie,
uitgevoerd in 38 Franse ziekenhuizen, includeerde 162 volwassen patiënten met
stadium IV of lokaal-recidiverend SCLC, en een ECOG performance status 2 of
beter. Ze werden gerandomiseerd naar carboplatine-etoposide (n=81) of topotecan
(n=81). Het primaire eindpunt was centraal-beoordeelde progressievrije
overleving. De mediane follow-up was 22,7 maanden (IQR 20,0-37,3). De mediane
PFS was 4,7 maanden in de combinatie-chemotherapiegroep versus 2,7 maanden in
de topotecangroep (gestratificeerd HR 0,57; p=0,0041). De meest-frequente graad
3 of 4 adverse events waren neutropenie (14% in de
combinatie-chemotherapiegroep versus 22% in de topotecangroep), trombocytopenie
(31% versus 36%), anemie (25% versus 21%), febriele neutropenie (6% versus
11%), en asthenie (9% versus 10%). Twee patiënten in de topotecangroep
overleden aan febriele neutropenie met sepsis. In de
combinatie-chemotherapiegroep overleed geen van de patiënten.
De
onderzoekers concluderen dat carboplatine-etoposide rechallenge een redelijke
tweedelijns optie was voor patiënten met gevoelig recidiverend SCLC.
