Definitieve
concurrente chemoradiotherapie (DCRT) wordt aanbevolen voor patiënten met
lokaal-gevorderd squameus celcarcinoom van de slokdarm (LA ESCC) die niet in
aanmerking komen voor chirurgie of chirurgie weigeren. Het optimale
chemotherapieregime voor deze patiënten is niet duidelijk. De multicenter fase
3-studie CRTCOESC in China heeft werkzaamheid van capecitabine, capecitabine
plus oxaliplatine (XELOX), en fluorouracil plus cisplatine (PF) als onderdeel
van DCRT voor LA ESCC vergeleken. Prof. Shegan Gao (Henan University of Science and Technology, Luoyang)
en collega’s publiceren de studie in het Journal of Clinical Oncology.1
De studie
includeerde 246 patiënten die werden gerandomiseerd naar twee cycli capecitabine
(n=80), XELOX (n=85), of PF (n=81) met concurrente IMRT. Binnen iedere arm
werden patiënten opnieuw gerandomiseerd naar al of niet twee cycli
consolidatiechemotherapie. De co-primaire eindpunten waren twee-jaars overall survival percentage en
incidentie van graad of hoger adverse
events. De twee-jaars OS-percentages waren 75% met capecitabine; 66,7% met
XELOX; en 70,9% met PF (verschillen niet significant) en de mediane OS in de
drie groepen was 40,9 maanden (95%-bti 34,4-49,9); 41,9 maanden (28,6-52,1); en
35,4 maanden (30,4-45,4). De incidentie van graad 3 of hoger AEs in de drie
groepen was 28,8%; 36,5%; en 45,7%. De mediane OS onder de patiënten die
consolidatiechemotherapie kregen was 41,9 maanden vergeleken met 36,9 maanden
onder de patiënten die geen consolidatiechemotherapie kregen (HR 0,71;
p=0,0403).
De
onderzoekers concluderen dat onder patiënten die DCRT kregen voor inoperabel LA
ESCC, capecitabine of XELOX niet resulteerden in significante verbetering van
de OS vergeleken met PF. Capecitabine was geassocieerd met lagere incidentie
van graad 3 of hoger AEs dan PF.

survival rate over fluorouracil plus cisplatin.
