Gedeeltelijke-borstbestraling (PBI) en intraoperatieve bestraling (IORT) zijn alternatieven voor gehele-borstbestraling (WBI) na borstsparende chirurgie voor vroeg-stadium mammacarcinoom (EBC). Een patiënt-gegevens meta-analyse van gepubliceerde fase 3-studies heeft uitkomsten na deze drie behandelingen geïnventariseerd. De analyse includeerde elf gerandomiseerde studies met tezaman 14.460 patiënten, onder wie 7675 (49,6%) WBI kregen terwijl 5413 (35,0%) en 2372 (15,3%) PBI respectievelijk IORT kregen. De mediane follow-up was 9 jaar. PBI was niet inferieur aan WBI voor de eindpunten ipsilateraal recidief (IBTR; HR 1,20; p=0,12), overall survival (OS; HR 1,02; p=0,70), en afstandsziektevrije overleving (DDFS; HR 1,15; p=0,43). IORT resulteerde in hoger IBTR-risico (HR 1,46; p<0,001) zonder verschillen voor OS (HR 0,98; p=0,81) of DDFS (HR 0,91; p=0,31).

De onderzoekers concluderen dat onder patiënten die borstsparende chirurgie ondergaan voor EBC, PBI niet inferieur was aan WBI voor alle drie de eindpunten, terwijl IORT inferieur was voor het eindpunt IBTR maar niet voor OS en DDFS.

Summary: Meta-analysis of 11 randomized phase 3 trials including 14,460 patients found that among patients undergoing breast-conserving surgery for early-stage breast cancer, partial breast irradiation was noninferior to whole-breast irradiation for the endpoint ipsilateral breast tumor recurrence, while intraoperative radiation was inferior to whole breast irradiation for this end point.
