De
standaard-behandeling voor locoregionaal gevorderd nasofarynxcarcinoom (NPC) is
platina-gebaseerde concurrente chemoradiotherapie (CRT).
Ma
en collega’s publiceren de studie ook online in The New England
Journal of Medicine.1
(inductiechemotherapiegroep, n=242) of alleen CRT (standaardtherapiegroep; n=238).
Het primaire eindpunt was recidiefvrije overleving; secundaire eindpunten waren
overall survival, veiligheid, en adherentie
aan de behandeling.
De mediane
follow-up was 42,7 maanden. De drie-jaars RFS was 85,3% in de
inductiechemotherapiegroep versus 76,5% in de standaardtherapie groep (HR 0,51;
p=0,001). De drie-jaars OS was 94,6% versus 90,3% (HR 0,43; 95%-bti 0,24-0,77).
De incidentie van graad 3 of 4 adverse
events was 75,7% in de inductiechemotherapiegroep versus 55,7% in de
standaardtherapiegroep.

Summary: A study in China and Singapore evaluated addition of gemcitabine plus cisplatin induction chemotherapy to the
standard concurrent platinum-based chemoradiotherapy for locally advanced
nasopharyngeal carcinoma. The three-year recurrence-free survival was 85.3% in
the induction chemotherapy plus chemoradiotherapy group versus 76.5% in the
chemoradiotherapy alone group (HR 0.51; p=0.001). Three-year OS was 94.6%
versus 90.3% (HR 0.43; 95% CI 0.24 to 0.77).
