Genetische prognostische en predictieve factoren in metastatisch colorectaalcarcinoom


Prof. Frederico InnocentiDe fase 3-studie CALGB/SWOG 80405 vond geen statistisch significant verschil in overall survival van patiënten die eerstelijns chemotherapie plus bevacizumab versus eerstelijns chemotherapie plus cetuximab kregen voor metastatisch colorectaalcarcinoom. Prof. Frederico Innocenti (University of North Carolina, Chapel Hill) en collega’s hebben nu primair-tumor DNA van 843 deelnemers van de studie geanalyseerd om genetische markers van OS te ontdekken. Ze publiceren hun resultaten online in het Journal of Clinical Oncology.1

De onderzoekers bepaalden genmutaties met PCR, microsatellietstatus door het genotyperen van microsatellieten, en tumormutatiebelasting (TMB) met next-generation sequencing. De analyses wezen uit dat patiënten met tumoren met hoge TMB langere OS hadden dan patiënten met lage TMB (HR 0,73; p=0,02). Onder patiënten met MSI-H tumoren was de OS langer in de bevacizumab-arm dan in de cetuximab-arm (HR 0,13; 95%-bti 0,06-0,30; p voor interactie tussen microsatellietstatus en behandelarm <0,001). Patiënten met BRAF-mutant tumoren hadden kortere OS dan patiënten met BRAF-wildtype tumoren (HR 2,01; p<0,001), en patiënten met RAS-mutant tumoren hadden kortere OS dan patiënten met RAS-wildtype tumoren (HR 1,52; p<0,001). De mediane OS was 35,9 maanden (95%-bti 33,0-38,8) onder patiënten met triple-negatieve tumoren (wildtype voor NRAS/KRAS/BRAF) versus 22,2 maanden (95%-bti 19,6-24,4) onder patiënten met tenminste één gemuteerd gen in hun tumoren (p<0,001).

De onderzoekers concluderen dat onder patiënten die eerstelijns behandeling kregen voor mCRC lage TMB en BRAF- en RAS-mutaties negatieve prognostische factoren waren. Patiënten met MSI-H tumoren hadden meer profijt van bevacizumab dan van cetuximab.

Summary: An analysis of primary tumor DNA of participants of the SWOG 80405 study showed that in patients with first-line treatment for mCRC, low tumor mutational burden and BRAF and RAS mutations are negative prognostic factors. Patients with MSI-H tumors had more benefit from bevacizumab than from cetuximab.
