Van zowel het
meervoudig onverzadigd vetzuur eisosapentaeenzuur (C20:5 n-3; EPA) als aspirine
is werkzaamheid gezien voor de chemopreventie van colorectaalcarcinoom, met een
uitstekend veiligheidsprofiel. De Engelse gerandomiseerde Systemic Evaluation of Aspirin and Fish Oil (SEAFOOD) Polyp Prevention
Trial onderzocht deze werkzaamheid in personen met coloscopisch
gedetecteerde sporadische colorectale neoplasie. Prof. Mark Hull (University of
Leeds UK) en collega’s publiceren de studie online in The Lancet.1
SEAFOOD werd
uitgevoerd in 53 centra in Engeland. De studie includeerde patiënten met high risk findings in het Engelse Bowel Cancer Screening Programme (drie
of meer adenomen waarvan tenminste één 10 mm of groter, of tenminste vijf
kleinere adenomen). Ze werden voor de duur van twaalf maanden 1:1:1:1
gerandomiseerd naar EPA 2 g per dag (n=179), aspirine 300 mg per dag (n=177),
EPA plus aspirine (n=177), of placebo (n=176). Het primaire eindpunt van de
studie was adenoma dection rate (ADR;
het percentage patiënten met tenminste één adenoom) bij de surveillance coloscopie
na een jaar interventie. De ADR was 63% in de EPA-groep, 61% in de
aspirinegroep, 61% in de combinatiegroep, en 61% in de placebogroep. EPA en
aspirine werden goed verdragen, hoewel het aantal gastroïntestinale adverse events verhoogd was in de
EPA-groep (146 AEs) vergeleken met 85 in de placebogroep, 86 in de
aspirinegroep en 68 in de combinatiegroep.
De
onderzoekers concluderen dat noch EPA noch aspirine noch de combinatie geassocieerd
was met lagere ADR in patiënten met coloscopische high risk
findings.

English randomised, double-blind, placebo-controlled 2 x 2 factorial SEAFOOD Polyp Prevention Trial randomized patients after colonoscopy with high
risk findings to EPA, aspirin, both EPA and aspirin, or placebo for 12 months.
The study found that neither EPA nor aspirin nor the combination were associated with a
reduction in the proportion of patients with at least one colorectal adenoma at
the surveillance colonoscopy after the intervention.
