Niet-eerder
behandelde patiënten met microsatelliet-stabiel metastatisch
colorectaalcarcinoom (mCRC) worden gewoonlijk behandeld met combinaties van
fluoruracil en folinezuur (FU/FA) plus oxaliplatine of irinotecan, en een EGFR-
of VEGF-gericht antilichaam. De Duitse multicenter gerandomiseerde
PANAMA-studie heeft toevoegen van panitumumab aan FU/FA-onderhoudstherapie voor
RAS-wildtype mCRC geëvalueerd. Prof.
Dominik Modest (Charité-Universitätsmedizin Berlin) en collega’s publiceren de studie in het Journal of Clinical
Oncology.1
De studie
includeerde patiënten die na eerstelijns inductietherapie met zes cycli FU/FA
en oxaliplatine plus pantitumumab stabiele ziekte of beter hadden. De patiënten
werden gerandomiseerd naar onderhoudstherapie panitumumab plus FU/FA (panitumumabgroep,
n=125) of alleen FU/FA (controlegroep, n=123). Het primaire eindpunt was
progressievrije overleving. Op het moment van data cutoff voor de nu
gepubliceerde analyse, na 218 PFS-gebeurtenissen, was de mediane PFS 8,8 maanden in de panitumumabgroep versus 5,7 maanden in de
controlegroep (HR 0,72; p=0,014). De overall
survival was numeriek maar niet statistisch significant langer in de
panitumumabgroep dan in de controlegroep (28,7 versus 25,7 maanden; p=0,32). De
ORR was 40,8% in de panitumumabgroep versus 26,0% in de controlegroep (OR 1,96;
p=0,02). De meest-frequente graad 3 of hoger adverse event was rash (7,2% van
de patiënten).
De
onderzoekers concluderen dat toevoegen van panitumumab aan
FU/FA-onderhoudstherapie voor RAS-wt
mCRC patiënten resulteert in significante verlenging van de PFS.

fluorouracil and folinic acid (FU/FA) in patients with at least stable disease
after induction therapy with panitumumab, oxaliplatin and FU/FA for RAS wild-type metastatic colorectal
cancer. The median PFS of maintenance therapy was 8.8
months with panitumumab and FU/FA versus 5.7 months with FU/FA alone (HR 0.72;
p=0.014) while the overall survival was numerically better with addition of
