De analyse includeerde 321 patiënten met indolent lymfoom (84% folliculair lymfoom, FL). Alle patiënten hadden bij inclusie (1998-1999 voor de eerste studie; 2002-2008 voor de tweede studie) eerstelijns behandeling gekregen met één of twee cycli van vier wekelijkse infusies van rituximab 375 mg/m2, en 148 patiënten waren gerandomiseerd naar toevoeging van interferon alfa-2a. De figuur toont de belangrijkste resultaten van de analyse (LSS: lymphoma-specific survival; TTF: time to treatment failure). Na mediaan 10,6 jaar follow-up (range 0,1 tot 18,8 jaar) waren 234 patiënten (73%) nog in leven. De tien- en vijftien-jaars OS waren 75% en 66%.
Onder alle patiënten had 36% (38% van de FL-patiënten) nooit chemotherapie nodig gehad. Onder FL-patiënten die binnen 24 maanden nieuwe therapie nodig hadden wegens vroege progressie van de ziekte was de tien-jaars OS 59%, versus 81% onder de patiënten met langer durende remissie. Toevoeging van interferon resulteerde niet in verbetering van de lange-termijn uitkomsten. Transformatie werd gezien in 20% van alle patiënten (2,4% per patiëntjaar) en in 18% van de FL-patiënten. Een nieuwe maligniteit werd gediagnostiseerd in 12%.
De onderzoekers concluderen dat ongeveer één op de drie patiënten met symptomatisch indolent lymfoom na eerstelijns rituximab zonder cytotoxische chemotherapie geen nieuwe behandeling nodig had tijdens tien jaar follow-up. De tien-jaars overleving in het cohort was excellent, zonder belangrijke safety issues.

Summary: Two Nordic Lymphoma Group trials treated advanced indolent lymphoma patients with first line rituximab-containing regimens without cytotoxic chemotherapy. An analysis after more than 10 years follow-up showed that approximately one third of the patients did not need new therapy. In the entire cohort the 10-year survival was excellent without major safety issues.
