Neurochirurgische resectie is een standaard lokale behandeling voor hersenmetastasen van longcarcinoom (LCBMs). Het is niet duidelijk of resectie overlevingsvoordeel bied onder patiënten met LCBMs en slechte Karnofsky performance status (KPS). Een retrospectieve cohortstudie in drie centra in China heeft de impact van lage pretreatment KPS op overall survival na resectie voor LCBMs geïnventariseerd. Prof. Yonggao Mou (Sun Yat-sen University Cancer Center, Guangzhou) en collega’s publiceren de studie in het Journal of Neuro-Oncology.¹

Deze figuur toont het flowdiagram van de studie. Onder de 1177 patiënten met LCBMs tussen juli 2010 en oktober 2022 in de drie centra waren er 386 met een pretreatment KPS 70 of lager. Van deze patiënten ondergingen 111 neurochirurgische resectie. Deze figuur laat zien dat de overall survival significant langer was in de groep met resectie dan in de groep zonder resectie (panel A; mediaan 14,7 versus 12,5 maanden; HR 0,68; p=0,01). Dit verschil bleef significant na propensity score matching (panel B; mediaan 17,7 versus 12,3 maanden; HR 0,61; p<0,01) en na inverse probability of treatment weighting (panel C; mediaan 17,7 versus 12,5 maanden; HR 0,58; p<0,01).

De onderzoekers concluderen dat ook onder LCBM-patiënten met lage KPS neurochirurgische resectie resulteerde in overlevingsvoordeel.
