Een
bijwerking van radiotherapie naar de hippocampus kan cognitiieve achteruitgang
zijn. Een studie van de Universiteit van Freiburg (Duitsland) heeft
oncologische uitkomsten geïnventariseerd van ontzien van de hippocampus tijdens
whole-brain radiation therapy met
simultaan geïntegreerde boost (HA-WBRT-SIB) voor patiënten met meerdere
hersenmetastasen. Prof. Anca Ligia Grosu en collega’s publiceren de studie online in Cancer.1
De studie
includeerde 66 patiënten die HA-WBRT (30 Gy in 12 fracties; dosering naar 98%
van het hippocampusvolume ten hoogste 9 Gy) plus SIB (51 of 42 Gy in 12
fracties) kregen voor metastasen of resectieholten, en patiënten die
conventionele WBRT kregen. Na propensity-score matching bestonden beide groepen
uit 62 patiënten. De lokale tumorcontrole van bestaande metastasen was
significant beter met HA-WBRT-SIB dan met conventionele WBRT (98% versus 82% na
één jaar; p=0,007), terwijl afstands-intracraniële controle beter was met
conventionele WBRT dan met HA-WBRT-SIB (82% versus 69% na één jaar; p=0,016).
De intracraniële progressievrije overleving (p=0,03) en de overall survival (p=0,001) waren significant beter in de
HA-WBRT-SIB groep. Vier patiënten in deze groep ontwikkelden
hippocampus-metastasen.
De
onderzoekers concluderen dat HA-WBRT-SIB een effectieve optie kan zijn voor
patiënten met meerdere hersenmetastasen. De impact op neurocognitie wordt nader
onderzocht in de lopende fase 2-studie HIPPORAD.

hippocampus-avoiding WBRT with a simultaneous integrated boost can be an
efficient therapeutic option for patients with multiple brain metastases, and
(compared with conventional WBRT) was associated with improved local tumor
control of existing metastases, higher intracranial PFS, a reduction of the
neurologic death rate, and an acceptable risk of radiation necrosis.
