Incrocci (Erasmus MC) en collega’s publiceren online in The Lancet Oncologyuitkomsten van de HYPRO-studie, een fase 3-studie van hypogefractioneerde
versus conventioneel gefractioneerde radiotherapie voor patiënten met gelokaliseerd
prostaatcarcinoom.1 De onderzoekers concluderen uit de studie dat
hypogefractioneerde radiotherapie voor het eindpunt vijf-jaars recidiefvrije
overleving niet superieur is aan conventionele radiotherapie, en niet kan
worden gezien als de nieuwe standaard van zorg voor patiënten met intermediair-
of hoog-risico prostaatcarcinoom.

Dearnaley (Institute of Cancer Research, Londen) en collega’s publiceren tegelijkertijd in The Lancet Oncologyuitkomsten van de CHHiP-studie, ook een fase 3-studie van hypogefractioneerde
versus conventioneel gefractioneerde radiotherapie voor prostaatcarcinoom.2
De onderzoekers concluderen uit de studie dat hypogefractioneerde
radiotherapie niet-inferieur is aan conventioneel gefractioneerde
radiotherapie, en dus moet worden gezien als de nieuwe standaard van zorg voor
gelokaliseerd prostaatcarcinoom.
Prof. Robert
Lee (Duke University, Durham NC) schrijft in een redactioneel commentaar in The Lancet
Oncology dat de verschillen tussen de conclusies samenhangen
met verschillen in opzet van de studies.3 HYPRO had een superiority design, en CHHiP een non-inferiority design. Vervolgens
schrijft hij dat hij het gedeeltelijk met beide conclusies eens is.
Behandelaars kunnen beide regimes met vertrouwen voorstellen aan hun patiënten.
