Er is geen
duidelijkheid over de prognostische waarde van immuunsuppressie in patiënten
met merkelcelcarcinoom (MCC). Een analyse van de National Cancer Database heeft de prognostische rol van immuunstatus
in gelokaliseerd MCC geïnventariseerd, en de relatie tussen IS en andere
prognostische factoren onderzocht. Dr. Mehran Yusuf (University of Louisville KY) en collega’s publiceren de

In de NCDB
over de periode begin 2010 tot eind 2016 identificeerden de onderzoekers patiënten
met gelokaliseerd MCC en bekende immuunstatus. De drie-jaars OS was 44,6% onder
de patiënten met immuunsuppressie versus 68,7% (p<0,0001) onder de
immuuncompetente patiënten. In multivariate analyse was immuunsuppressie geassocieerd
met hogere mortaliteit (gecorrigeerd HR 2,36; 95%-bti 2,03-2,75). De etiologie
van immuunsuppressie was geassocieerd met OS (p=0,0015), en patiënten met
solide-orgaantransplantatie hadden met 32,7% de laagste drie-jaars OS. Immuunsuppressie
was geassocieerd met verhoogd risico van hogere nodale belasting (OR 1,70;
95%-bti 1,37-2,11) en lymfovasculaire invasie (OR 1,58; 95%-bti 1,23-2.03).
De
onderzoekers concluderen dat immuunstatus onafhankelijk prognostisch was voor
OS van patiënten met gelokaliseerd MCC.

Summary: Analysis of the National Cancer Database found that among patients with Merkel
cell carcinoma, immune status was independently prognostic for overall
survival. The etiology of immune suppression may be associated with differential
survival outcome. Multiple adverse prognostic factors were associated with
increased likelihood of immune suppression.
