De onderzoekers bepaalden het PAM50-gebaseerde subtype van 672 tumoren van patiënten die endocriene therapie plus ribociclib hadden gekregen en 488 tumoren van patiënten die endocriene therapie plus placebo hadden gekregen. De subtypedistributie was luminal A 46,7%; luminal B 24,0%; normal-like 14,0%; HER2-verrijkt 12,7%; en basal-like 2,6%. Deze distributie was over het algemeen consistent over de behandelarmen en de drie studies. Het risico van ziekteprogressie, vergeleken met luminal A, was respectievelijk 1,44; 2,31; en 3,96 maal hoger voor luminal B; HER2-verrijkt; en basal-like (p<0,001). Alle subtypen behalve basal-like hadden significant PFS-profijt van toevoegen van ribociclib aan endocriene therapie: HER2-verrijkt HR 0,39 (p<0,0001), luminal B HR 0,52; p<0,001), luminal A HR 0,63 (p=0,0007), normal-like HR 0,47 (p=0,0005), en basal-like HR 1,15 (p=0,77).
De onderzoekers concluderen dat de intrinsieke subtypen sterk en consistent geassocieerd waren met progressievrije overleving op endocriene therapie. In alle subtypen behalve basal-like werd profijt gezien van toevoegen van ribociclib aan endocriene therapie.

Summary: A retrospective exploratory analysis of three MONALEESA studies (endocrine therapy plus ribociclib or placebo for HR-positive, HER2-negative advanced breast cancer) evaluated the association of intrinsic subtypes with prognosis and treatment benefit. The intrinsic subtypes were strongly and independently associated with progression-free survival on endocrine-based therapy. Patients with luminal A, luminal B, HER2-enriched, and normal-like subtypes had consistent benefit from addition of ribociclib to endocrine therapy, while patients with basal-like subtype did not benefit from ribociclib.
