De
multinationale fase 3-studie KEYNOTE-048 randomiseerde patiënten met
recidiverend of metastatisch squameus celcarcinoom van hoofd en hals (R/M
HNSCC) 1:1:1 naar alleen pembrolizumab, pembrolizumab met chemotherapie, of cetuximab
met chemotherapie. In 2019 is gepubliceerd dat pembrolizumab monotherapie
vergeleken met cetuximab plus chemotherapie geassocieerd was met betere overall
survival onder de patiënten met PD-L1 combined
positive score (CPS) 20 of hoger en onder patiënten met CPS 1 of hoger,
terwijl pembrolizumab plus chemotherapie vergeleken met cetuximab plus
chemotherapie geassocieerd was met betere OS onder alle patiënten. Dr. Barbara
Burtness (Yale University School of Medicine, New Haven CT) en collega’s publiceren nu in het Journal of Clinical
Oncology een post-hoc analyse van de uitkomsten in de subgroepen met CPS
lager dan 1 en met CPS 1 tot 20.1
Onder de 882
KEYNOTE-048 patiënten waren er 128 met PD-L1 CPS < 1 en 373 met CPS 1-20.
Met pembrolizumab monotherapie versus cetuximab plus chemotherapie was de
mediane OS 7,9 maanden versus 11,3 maanden in de PD-L1 CPS <1 subgroep (HR
1,51; 95%-bti 0,96-2,37) en 10,8 versus 10,1 maanden in de PD-L1 CPS 1-20 subgroep
(0,86; 0,66-1,12). Met pembrolizumab plus chemotherapie versus cetuximab plus
chemotherapie was de mediane OS 11,3 versus 10,7 maanden in de PD-L1 CPS < 1
subgroep (HR 1,21; 95%-bti 0,76-1,94) en 12,7 versus 9,9 maanden in de CPS 1-20
subgroep (0,71; 0,54-0,94).
De onderzoekers
concluderen dat over het algemeen de werkzaamheid van pembrolizumab
monotherapie en van pembrolizumab plus chemotherapie toenam met toename van de
PD-L1 expressie. Deze expressie is een bruikbare biomarker voor werkzaamheid
van pembrolizumab, maar in de groep met lage expressie is er behoefte aan
additionele biomarkers.

Summary: The
multinational phase 3 KEYNOTE-048 trial randomized patients with recurrent or
metastatic head and neck squamous cell carcinoma (R/M HNSCC) 1:1:1 to
pembrolizumab alone, pembrolizumab plus chemotherapy, or cetuximab plus
chemotherapy. Post-hoc analyses of the study show that pembrolizumab and
pembrolizumab-chemotherapy had antitumor activity in the PD L1 combined
positive score (CPS) 1-19 subgroup, with pembrolizumab-chemotherapy leading to
numerically longer overall survival than cetuximab-chemotherapy. In the PD-L1
CPS < 1 subgroup, neither pembrolizumab nor pembrolizumab chemotherapy
demonstrated improvement in overall survival compared with
cetuximab-chemotherapy.
