Het
multinationale T Cell Project was de tot
op heden grootste prospectieve studie van incidentie, behandelpatronen, en
uitkomsten van T-cel lymfomen. Zeldzame subtypen van T-cel lymfoom, zoals
hepatosplenisch (HSTCL), enteropathie-geassocieerd (EATL), en perifeer gamma
delta (PGDTCL) T-cel lymfomen, worden zelden geïncludeerd in studies. Een
analyse van het T Cell Project heeft
behandelpatronen en uitkomsten van deze subtypen geïnventariseerd. Prof. Francine Foss (Yale University
School of Medicine, New Haven CT) en collega’s
publiceren de analyse online in het American Journal of Hematology.1
De analyse
includeerde 31 patiënten met HSTCL, 65 patiënten met EATL, en 19 patiënten met
PGDTCL. De meest-gebruikte eerstelijns behandeling was anthracycline-gebaseerde
chemotherapie, met response rates van
20-40%. Autologe stamceltransplantatie als consolidatie in eerste remissie werd
toegepast in 33% van de HSTCL-patiënten, 7% van de EATL-patiënten, en 12% van
de PGDTCL-patiënten, terwijl vier HSTCL-patiënten in eerste remissie allogene
stamceltransplantatie ondergingen. De progressievrije overleving na drie jaar
liep voor de drie subtypen uiteen van 28% tot 40%, en de drie-jaars overall survival was met 70% het meest
gunstig voor PGDCTL.
De
onderzoekers concluderen dat er behoefte is aan nieuwe behandelopties voor deze
zeldzame subtypen van T-cel lymfoom.
