Incorporatie van functionele status, fragiliteit, comorbiditeit, en comedicatie in overlevingsmodellen voor CRC

Dr. Ben SchöttkerOnder oudere patiënten met colorectaalcarcinoom (CRC) worden vaak beperkingen in functionele status, comorbiditeiten, en gebruik van comedicatie gezien. Een analyse onder deelnemers van de Duitse DACHS (Darmkrebs: Chancen der Verhütung durch Screening)-studie heeft de waarde geïnventariseerd van incorporatie van deze factoren in een overlevingsmodel voor CRC-patiënten. Dr. Ben Schöttker (Deutsches Krebsforschungszentrum, Heidelberg) en collega’s publiceren de analyse in het International Journal of Cancer.1

De analyse includeerde 3410 CRC-patiënten. Het referentie-voorspellingsmodel omvatte de factoren leeftijd, geslacht, tumorstadium, en tumorlocatie. Voor de voorspelling van vijf-jaars overall survival resulteerde toevoeging van functionele status in het beste model onder alle CRC-patiënten (AUC 0,762) en onder patiënten jonger dan 65 jaar (AUC 0,820); terwijl onder patiënten van 65 jaar en ouder toevoeging van ook comorbiditeit resulteerde in het beste model (AUC 0,747). Voor het eindpunt vijf-jaars niet-ziektespecifieke overleving had het referentiemodel plus comorbiditeit en fragiliteit de beste voorspellende performance onder alle CRC-patiënten (AUC 0,776). Voor de uitkomsten vijf-jaars ziektevrije overleving (AUC 0,727), ziektespecifieke overleving (AUC 0,838), en recidiefvrije overleving (AUC 0,784) was het referentiemodel zonder toevoegingen al het beste model.

De onderzoekers concluderen dat hoe minder CRC-specifiek de overlevingsuitkomst is en hoe ouder de patiënt, des te meer relevant inclusie is van functionele status, comorbiditeit, en fragiliteit in het voorspellingsmodel.

Summary: Analysis of 3,410 colorectal cancer patients participating in the German DACHS study found that the less CRC-specific the survival outcome and the older the patient, the more relevant was the inclusion of functional status, comorbidity, and frailty in CRC prognostic scores.
